Often technology and medical equipment are not configured to address the unique dosing, dilutions, standard concentrations, and infusion rates for pediatric patients. These investments in procedures and protocols to support clinical decision making are usually limited to Children's hospitals. But, the reality is that most children will be seen in facilities focused on the care of adult patients. They don't have the comprehensive safety measures and infrastructure to prevent common dosing errors or onsite staff pediatric pharmacists to review every order.

The Pediatric Prescriber's Handbook provides guided dosing to aid in the challenge of the unique issues associated with pediatric medication dosing. Unlike adult medicine that primarily uses fixed dosing, most pediatric medications are weight-based, and require manipulation/dilution of commercially available products intended for use in adult patients.

Guided dosing utilizes weight-based dosing, i.e. "mg/kg/dose" in the framework of initial dosing, recommended titration intervals, as well as maximum and maintenance doses. It provides references for the prescriber to not exceed the "Adult initial dose" or "Adult Max dose". Pediatric dosages (mg/kg/dose), should not exceed adult dosages (mg/dose). Having these "visual" reference points, will serve as a double check and aid in avoiding 10-fold calculation errors.
